Your employer has provided a range of participation options for you to choose from to meet its wellness program requirements. To submit your attestation of completion for a wellness elective, simply scroll to the elective and complete the corresponding attestation form by clicking the link.

Participation at an Agency Sponsored Health and Wellness related event. If you are not sure if your event, class, course, or program would qualify, contact Lisa Roberts in HR at (845) 673-7118 for clarification and/or more information.

Volunteering for a Cause

Engagement in an activity willingly and without pay for the benefit of others. There is no minimum amount of time that must be spent engaged in service to successfully meet completion requirement. Wellness Workshop

Attendance and engagement in a minimum of one (1) in-person or on-line wellness related education class, course or program. InDeed WellnessTalks are available in your member portal and require the successful completion of a short quiz for credit.

Attend Lifeworks Webinars

Attendance and engagement in a minimum of two (2) Friday Bi-Weekly LifeWorks Wellbeing Webinars.

Current Wellbeing calendar

When submitting your elective, be sure to include the completion date and name of the webinar you completed.

Stress Management Class

Attendance and engagement in a minimum of one (1) in-person or on-line certified stress reduction/management related education class, course, or program. If you are not sure if your class/course/program would qualify, contact Lisa Roberts in HR at (845) 673-7118 for clarification and/or more information.

 

Complete a Nutrition Course

Attendance and engagement in a minimum of one (1) in-person or on-line nutrition education class, course, or program. If you are not sure if your class/course/program would qualify, contact Lisa Roberts in HR at (845) 673-7118 for clarification and/or more information.

Suicide Prevention Training

Participate in Sports League or Club

Regular and active participation in an organized sports league or physical group activity (i.e., racquet ball league, soccer league, bowling/ golf league, yoga or swimming classes, etc.). Participation, in this instance, is defined as regular attendance for a period of time and engagement in physical activities. If you are not sure if your group would qualify, contact Lisa Roberts in HR at (845) 673-7118 for clarification and/or more information.

Online Fitness Tracking Program

Four (4) consecutive months of at least two (2) times per week of physical activity tracked via an online program, smartphone app or fitbit. If using fitbit or another pedometer tracker, average steps per month to equal 250,000. Programs could include Nike Run, My Fitness Pal, Fitbit, etc. If you are not sure if your program tracker would qualify, contact Lisa Roberts in HR at (845) 673-7118 for clarification and/or more information.

Physical Activity for a Cause

Participation in a physical activity to raise awareness and/or money for a cause. There is no minimum distance or time that must be completed to successfully meet completion requirement. If you are not sure if your physical activity would qualify, contact Lisa Roberts in HR at (845) 673-7118 for clarification and/or more information.

Active Gym Membership

Four (4) consecutive months of active gym membership (i.e., Gold’s Gym, Planet Fitness, YMCA, YWCA, etc.), regular attendance and engagement in physical activities at least two (2) times per week. Complete a new or annual vaccination

Vaccines include COVID/Booster, Flu, Pneumonia, Shingles, Hep-B received in the current wellness year. Contact Lisa Roberts in HR at (845) 673-7118 for clarification and/or more information.

Complete a Well Visit

Go for an annual, preventative well visit (PCP, Dentist, Optometrist) or preventative cancer screening between November 1 and October 31 of the current year. Contact Lisa Roberts in HR at (845) 673-7118 for clarification and/or more information.
